Job summary

Arbor Care Centers - Valhaven is seeking a Licensed Practical Nurse (LPN) to provide direct and indirect nursing care to adults and geriatrics. You will supervise CNAs, collect care data, and contribute to care and discharge planning while collaborating with the health team to ensure smooth care transitions.

Qualifications

• Current state license to practice as a LPN or RN.
• Must maintain current provider CPR certification throughout employment.

• Contribute to the development of the plan of care and discharge plan for the assigned patient.
• Implement patient care utilizing care plan, delegated medical orders, and collaborate with health team members as necessary.
• Assist in evaluating outcomes of patient care planning and teaching and in revising the plan according to changing needs of the patient.
• Participate in the orientation and education of personnel and students.
• Document the delivery of health care and nursing process in accordance with unit standards and hospital policy.

About the Role:

As an LPN at Arbor Care Centers - Valhaven, you will be responsible for providing direct and indirect nursing care to patients ranging in age from 18 years to geriatric. You will work closely with our interdisciplinary team to develop and implement individualized care plans, while promoting a positive and supportive environment for our patients and their families. Your exceptional communication and interpersonal skills will enable you to effectively collaborate with physicians, Certified Nurses Aides, Medication Aides, and other healthcare professionals to ensure seamless care transitions.
