Job summary

Arbor Care Centers - Valhaven is seeking a Licensed Practical Nurse (LPN) to provide direct and indirect nursing care to adults and geriatrics. You will supervise CNAs, collect care data, and contribute to care and discharge planning while collaborating with the health team to ensure smooth care transitions.

You will document护理 delivery, participate in ongoing education of staff, and support care plan revisions as patient needs evolve. Competitive hourly pay and benefits offered.

Qualifications

• Current state license to practice as a LPN or RN.
• Must maintain current provider CPR certification throughout employment.

Responsibilities

• Supervise Certified Nurses Aides and Medication Aides.
• Collect patient care data and monitor patient conditions and family dynamics.
• Contribute to the development of the plan of care and discharge plan for the assigned patient.
• Implement patient care utilizing care plan, delegated medical orders, and collaborate with health team members as necessary.
• Assist in evaluating outcomes of patient care planning and teaching and in revising the plan according to changing needs of the patient.
• Participate in the orientation and education of personnel and students.
• Document the delivery of health care and nursing process in accordance with unit standards and hospital policy.
