Job summary

Coastal Community Health Services (CCHS) is seeking an LPN to join our team in Coastal Georgia. In an outpatient primary care setting, you will perform nursing assessments, administer medications, take vital signs, and support care coordination under the supervision of providers.

We value compassionate patient care, strong communication, and teamwork. Eligible candidates hold a Practical Nursing diploma, Georgia LPN license, BLS/CPR, and 1–3 years of outpatient experience; bilingual

Qualifications

  • Degree or diploma in Practical Nursing from an accredited nursing program.
  • Current Georgia LPN license.
  • Current BLS/CPR certification.
  • 1–3 years of nursing experience in outpatient/primary care preferred.
  • Knowledge of nursing assessment, medication administration, injections, immunizations, and other clinical procedures within the LPN scope.
  • Strong communication, organization, and patient education skills.
  • Ability to recognize and communicate changes in a patient’s condition and respond appropriately.
  • Ability to use an Electronic Health Record (EHR) system.
  • Ability to work collaboratively with a healthcare team.
  • Bilingual in English and Spanish is a plus.

Responsibilities

  • Perform nursing assessments and obtain vital signs, health histories, and other clinical information.
  • Perform routine tests and screenings and communicate findings to providers.
  • Administer medications, treatments, injections, and immunizations as ordered.
  • Assist providers with patient examinations and procedures.
  • Triaging patient calls and visits within the LPN scope of practice.
  • Provide patients and caregivers with education regarding medications, treatment plans, and preventive care.
  • Coordinate referrals, testing, procedures, appointments, and prior authorizations.
  • Support patients with complex healthcare needs through care coordination and care management.
  • Maintain accurate and complete clinical documentation.
  • Review immunization status and document immunizations.
  • Collaborate with providers and staff to support continuity of care.
  • Participate in Quality Improvement activities and clinical standards.

What You’ll Do
  • Performing nursing assessments and obtaining vital signs, health histories, and other clinical information

  • Coordinating specialist referrals, testing, procedures, appointments, and prior authorizations
  • Supporting patients with complex healthcare needs through care coordination and care management activities
  • Maintaining accurate and complete clinical documentation
  • Reviewing immunization status and administering and documenting immunizations
  • Collaborating with providers, clinical staff, patients, caregivers, and outside healthcare facilities to support continuity of care
  • Participating in Quality Improvement activities and supporting CCHS clinical standards and protocols
What We’re Looking For
  • Degree or diploma in Practical Nursing from an accredited nursing program
  • Current and valid Licensed Practical Nurse (LPN) license in the state of Georgia
  • Current BLS/CPR certification
  • One to three years of nursing experience in an outpatient or primary care setting preferred
  • Knowledge of nursing assessment, medication administration, injections, immunizations, and other clinical procedures within the LPN scope of practice
  • Strong communication, organization, and patient education skills
  • Ability to recognize and communicate changes in a patient's condition and respond appropriately
  • Ability to use an Electronic Health Record (EHR) system
  • Ability to work collaboratively as part of a healthcare team
  • Bilingual in English and Spanish is a plus
Clinic Location and Hours:

Monday - Friday: 8:00am - 5:00pm
