Job summary

Akkermans Leisure & Golf seeks a Licensed Practical Nurse (LPN) to lead the health and wellbeing of residents in the community. The role covers overnight, 12-hour shifts, ensuring high-quality clinical care and regulatory compliance.

You will supervise Certified Nursing Assistants and direct care staff, coordinate with the administrator, and partner with a pharmacy consultant for optimal medication services. Strong assessment, documentation, triage, and communication skills are essential.

Qualifications

• Maintains current state LPN license.
• Experience in assisted living, home health, or long term care environments.
• Two (2) years experience as an LPN.
• Demonstrates competence in assessment, injections/medication administration, follow up, charting, triage, communication, and effective team leading/supervision.

Responsibilities

• Maintains communications with healthcare providers of changes or requests for orders.
• Ensures compliance with federal, state, and local laws and regulations pertaining to clinical care.
• Collects and reports occurrences and assists in improvement plans.
• Partners with Administrator and team to maintain resident and staff health safety.
• Collaborates with pharmacy consultant to provide optimal pharmaceutical services.
• Responsible for infection control practices in the community.
• Oversees safe distribution/administration of medications and injections when needed.
• Identifies resident needs via service plan process and documents changes in health status.
• Finishes assigned evaluations.
• Works with other departments to review service/medication levels and update service plans as per regulations.
• Maintains communications with residents' families regarding care changes.

Position Summary

Responsible for providing overall leadership and management of the health and wellbeing of the residents within the community.

Essential Duties And Responsibilities

• Maintains communications with Healthcare Providers of recent changes or requests for orders.
• Ensures that community follows all federal, state, local laws and regulations as it pertains to clinical care.
• Responsible for collecting, and reporting occurrences, and if necessary, assist in developing/implementing improvement plans.
• Partners with Administrator and other team members to maintain resident and team member health safety.
• Partners with pharmacy consultant to provide optimal pharmaceutical services to residents.
• Responsible for infection control practices in community.
• Responsible for the safe and efficient distribution/administration of medications, injections, nebulizers, moderate wound care per state regulations, as needed when floor nurse is not available.
• Identifies ongoing needs and services of residents through the evaluation/ Service Plan process including documentation for residents with a change in health care status.
• Responsible for the completion of any assigned evaluations.
• Partners with other department coordinators to identify, review, and discuss potential change in service/medication levels and updates the resident’s service plan as indicated by state regulations.
• Maintains communications with resident’s family and/or responsible party regarding changes in care or health concerns.
• Other duties as assigned.

Supervisory Responsibilities

Provides supervision to Certified Nursing Assistants and all direct care staff as assigned to assure delivery of consistently high quality care to residents. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

Educational Requirements And Experience

• Maintains current state license as a Licensed Practical Nurse per state regulations.
• Experience in assisted living, home health, or long term care industries.
• Two (2) years experience as a LPN.
• Must demonstrate competence in assessment skills, injections/medication administration, follow up, charting, triage, communication, and effective team leading/supervision.
