Job summary

Akkermans Leisure & Golf is seeking an LPN to lead health and wellbeing services for residents in a live-in community. Overnight 12-hour shifts are available, with a focus on safe medication administration, pain management, and coordination with healthcare providers.

Responsibilities include supervising care staff, maintaining documentation, and ensuring regulatory compliance across the care continuum. Nursing licensure and 2+ years in relevant settings are required.

Qualifications

• Current LPN license in Florida.
• Two years of experience in assisted living, home health, or long-term care.
• Competence in injections/medication administration, charting, triage, and effective team leadership.

Responsibilities

• Maintains communications with Healthcare Providers regarding orders and changes.
• Ensures compliance with federal, state, local laws and regulations governing clinical care.
• Collects and reports occurrences and assists in improvement plans.
• Partners with Administrator and team to maintain resident health safety.

POSITION SUMMARY

Responsible for providing overall leadership and management of the health and wellbeing of the residents within the community.

SHIFT: Overnight12-hour shifts

ESSENTIAL DUTIES AND RESPONSIBILITIES

The following duties are normal for this position. This list is not to be construed as exclusive or all inclusive. Other duties may be required and assigned.

• Maintains communications with Healthcare Providers of recent changes or requests for orders.
• Ensures that community follows all federal, state, local laws and regulations as it pertains to clinical care.
• Responsible for collecting, and reporting occurrences, and if necessary, assist in developing/implementing improvement plans.
• Partners with Administrator and other team members to maintain resident and team member health safety.
• Partners with pharmacy consultant to provide optimal pharmaceutical services to residents.
• Responsible for infection control practices in community.
• Responsible for the safe and efficient distribution/administration of medications, injections, nebulizers, moderate wound care per state regulations, as needed when floor nurse is not available.
• Identifies ongoing needs and services of residents through the evaluation/ Service Plan process including documentation for residents with a change in health care status.
• Responsible for the completion of any assigned evaluations.
• Partners with other department coordinators to identify, review, and discuss potential change in service/medication levels and updates the resident’s service plan as indicated by state regulations.
• Maintains communications with resident’s family and/or responsible party regarding changes in care or health concerns.
• Other duties as assigned.

SUPERVISORY RESPONSIBILITIES

Provides supervision to Certified Nursing Assistants and all direct care staff as assigned to assure delivery of consistently high quality care to residents. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

Educational Requirements and Experience

• Maintains current state license as a Licensed Practical Nurse per state regulations.
• Experience in assisted living, home health, or long term care industries.
• Two (2) years experience as a LPN.
• Must demonstrate competence in assessment skills, injections/medication administration, follow up, charting, triage, communication, and effective team leading/supervision.
