Job summary

The Grove at Countryside in Clearwater, FL seeks an experienced Licensed Practical Nurse to lead the health and wellbeing of residents. You will supervise CNAs, coordinate care with physicians, and ensure safe medication administration.

This is a hands-on leadership position focused on compassionate, compliant care delivery. The role requires a Florida LPN license and at least two years in assisted living or long-term care, with strong assessment, communication, and leadership skills.

Qualifications

• Must maintain current LPN license in Florida.
• 2 years’ experience as LPN in assisted living, home health, or long-term care.
• Competence in assessment, injections/medication administration, charting, triage, communication, and effective team leading/supervision.

Responsibilities

• Oversee nursing care and coordinate with healthcare providers.
• Ensure compliance with federal, state, and local laws and regulations governing clinical care.
• Collect, report occurrences, and assist in developing improvement plans.
• Partner with Administrator and team to maintain resident and staff health safety.
• Coordinate with pharmacy consultant to provide optimal pharmaceutical services.
• Ensure infection control practices and safe distribution/administration of medications.
• Identify ongoing needs and update resident service plans per state regulations.
• Maintain communications with residents' families about changes in care or health concerns.
• Supervise CNAs and direct care staff to ensure high‑quality care.

Responsible for providing overall leadership and management of the health and wellbeing of the residents within the community.

Essential Duties and Responsibilities

• Maintains communications with Healthcare Providers of recent changes or requests for orders.
• Ensures that community follows all federal, state, local laws and regulations as it pertains to clinical care.
• Responsible for collecting, and reporting occurrences, and if necessary, assist in developing/implementing improvement plans.
• Partners with Administrator and other team members to maintain resident and team member health safety.
• Partners with pharmacy consultant to provide optimal pharmaceutical services to residents.
• Responsible for infection control practices in community.
• Responsible for the safe and efficient distribution/administration of medications, injections, nebulizers, moderate wound care per state regulations, as needed when floor nurse is not available.
• Identifies ongoing needs and services of residents through the evaluation/ Service Plan process including documentation for residents with a change in health care status.
• Responsible for the completion of any assigned evaluations.
• Partners with other department coordinators to identify, review, and discuss potential change in service/medication levels and updates the resident’s service plan as indicated by state regulations.
• Maintains communications with resident’s family and/or responsible party regarding changes in care or health concerns.
• Other duties as assigned.

Supervisory Responsibilities

Provides supervision to Certified Nursing Assistants and all direct care staff as assigned to assure delivery of consistently high-quality care to residents. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws.

Educational Requirements and Experience

• Maintains current state license as a Licensed Practical Nurse per state regulations.
• Experience in assisted living, home health, or long-term care industries.
• Two (2) year's experience as LPN.
• Must demonstrate competence in assessment skills, injections/medication administration, follow up, charting, triage, communication, and effective team leading/supervision.
