Job summary

Andwell Health Partners is seeking an LPN to join our hospice team in Lewiston, ME. You will follow care plans, educate patients and families, and support independence in home and community settings.

Responsibilities include nursing assessments, safety evaluations, and collaborative care planning with RNs and healthcare teams. This role emphasizes compassionate, patient-centered care in a flexible home-based environment.

Qualifications

• Graduate of an accredited NLN School for Practical Nursing.
• 2 to 3 years of recent acute care nursing or equivalent experience; community health nursing desirable.
• Current LPN license in the State of Maine & CPR Certification.
• Must have reliable transportation with insurance and a current Maine Driver's license.

Responsibilities

• Follow prescribed plan of care developed by the primary RN.
• Provide patient/caretaker with adequate knowledge to make informed decisions and judgments by teaching preventive care and helping family identify health problems.
• Promote patient/family independence in accepting responsibility for care.
• Provide supportive physical, emotional and spiritual care.
• Evaluate home/family safety. Make recommendations for improvements and elimination of hazards.
• Utilize the nursing process in implementing plan of care for patients and their caregivers.
• Contribute to the assessment of community health needs, health education, and participate in preventive health activities.
