Job summary

Emerald Care Center Midwest in Midwest City, OK seeks a Licensed Practical Nurse (LPN) to join our Nursing team. You will provide direct patient care, administer medications, monitor residents, and collaborate with the interdisciplinary team to implement care plans.

Growth and ongoing education are emphasized in a supportive work environment. This role requires flexibility across shift times (6am–6pm, 6pm–6am) and PRN coverage, with a focus on compassionate, patient‑centered care and teamwork

Responsibilities

• Provide direct patient care to residents, including administration of medications, wound care, and tracheostomy management
• Conduct vital signs, monitor for changes in condition, and report observations to nursing staff
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Participate in care conferences, rounds, and quality improvement initiatives to ensure seamless coordination of care
• Maintain accurate and detailed records of patient care and medication administration
• Collaborate with other healthcare professionals to ensure patient needs are met, and positive outcomes are achieved
