Job summary

Emerald Nursing & Rehab Omaha in Omaha, NE is seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our Nursing department. You will assess patient needs, provide direct nursing care, administer medications, and collaborate with the healthcare team to ensure high-quality resident care.

The role supports a skilled nursing facility environment with shift options (6a-6p, 6p-6a, 6a-2p, 2p-10p, 10p-6a).

Qualifications

• Licensure as LPN/LVN in Nebraska or multi-state is required.
• Graduation from an accredited LPN/LVN program is required.
• Experience in a skilled nursing facility or similar setting preferred.
• Current CPR/BLS certification required; additional state certifications may be required.
• Excellent verbal and written communication; ability to work with a diverse care team.

Responsibilities

• Provide direct nursing care to residents, including medications, wound care, and treatments as prescribed.
• Monitor vital signs, document progress, and report significant changes to the Charge Nurse or DON.
• Assist in developing and implementing individualized care plans for residents.
• Maintain clear and accurate documentation of patient care; communicate with residents, families, and the healthcare team.
• Adhere to facility policies and infection control and safety protocols.
• Provide emotional support and education to residents and families about care plans and health conditions.
• Collaborate with RNs, CNAs, physicians, and therapists to deliver coordinated care.

• Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
• Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
• Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
• Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
• Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.
