Job summary

As a Licensed Practical Nurse (LPN)/ Registered Nurse (RN) at Emerald Nursing & Rehab Omaha, you will deliver high-quality, compassionate care to our residents. Qualifications

• LPN/LVN license Nebraska or multi-state required.
• Graduation from an accredited LPN/LVN program.
• Previous skilled nursing or similar healthcare experience preferred.
• Current CPR/BLS certification required; additional certifications may be needed by state.
• Excellent verbal and written communication; ability to work with a diverse team.
• Physical ability to lift/move patients and stand for long periods.

Responsibilities

• Provide direct nursing care to residents, including medications, wound care, and treatments as prescribed.
• Monitor residents' vital signs, document progress, and report changes to the Charge Nurse or DON.
• Assist in developing and implementing individualized care plans.
• Maintain clear, accurate documentation and communicate with residents, families, and team.
• Adhere to facility policies, infection control, and safety procedures.
• Provide emotional support and education to residents and families.
• Collaborate with RNs, CNAs, physicians, and therapists for coordinated care.

• Patient Care: Provide direct nursing care to residents, including administering medications, wound care, and treatments as prescribed by the physician.
• Monitoring and Assessment: Monitor residents’ vital signs, document their progress, and report any significant changes to the Charge Nurse or Director of Nursing.
• Care Planning: Assist in the development and implementation of individualized care plans, ensuring they are tailored to meet each resident’s needs.
• Communication: Maintain clear and accurate documentation of patient care, communicate effectively with residents, families, and the healthcare team.
• Compliance: Adhere to all facility policies, procedures, and regulatory requirements, including infection control and safety protocols.
• Support: Provide emotional support and education to residents and their families, helping them understand care plans and health conditions.
• Team Collaboration: Work collaboratively with other healthcare professionals, including RNs, CNAs, physicians, and therapists, to deliver coordinated and comprehensive care.

Qualifications

• Licensure: Valid LPN/LVN license in Nebraska or multi-state.
• Education: Graduation from an accredited LPN/LVN program.
• Experience: Previous experience in a skilled nursing facility or similar healthcare setting preferred.
• Certifications: Current CPR/BLS certification required. Additional certifications may be required based on state regulations.
• Communications: Excellent verbal and written communication skills. Ability to work collaboratively with a diverse team of healthcare professionals and build positive relationships with residents and their families.
