Tennessee Quality Care is hiring a Licensed Practical Nurse (LPN) for full-time Home Health in Shelby County, TN. You’ll deliver compassionate home-based care, participate in care planning, and educate patients and families to promote independence.

Qualifications

• Active LPN/LVN license in the state of Tennessee.
• Graduate of an accredited practical/vocational nursing program.
• 3+ years of nursing experience (Home Health, Med-Surg, or Community Health preferred).
• Current CPR certification is required.
• Reliable transportation, valid driver’s license, and auto insurance.

Responsibilities

• Deliver direct patient care in accordance with the state Nurse Practice Act.
• Carry out care plans developed by the Registered Nurse (RN).
• Educate patients and families on self-care and treatment plans.
• Document care accurately and timely to maintain records.
• Coordinate with RN and care team to ensure comprehensive home care support.
• Utilize proper techniques for equipment, supplies, and infection control.
• Support patients’ physical, emotional, and educational needs in the home.
