Tennessee Quality Care in Dover, TN is seeking a dedicated Licensed Practical Nurse (LPN) for full-time home health visits in Dover and surrounding areas. Join a team that values compassionate care, patient independence, and one-to-one support in the comfort of patients’ homes.

You will follow a plan of care, document accurately, educate patients and families, and coordinate with the RN and other disciplines to deliver high-quality home health services.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Has three years nursing experience; community/home health or medical-surgical experience preferred.
• Currently licensed as an LPN or LVN in the state.
• Must have reliable transportation, valid driver's license, and automobile insurance.
• CPR certification is required.

Responsibilities

• Provide direct patient care as defined in the state Nurse Practice Act.
• Implement plan of care initiated by the registered nurse.
• Provide accurate and timely documentation per the plan of care.
• Educate patient and family/caregiver on self-care and information pertinent to diagnosis and plan of care.
• Coordinate home health services with other disciplines and report identified needs to the RN/Clinical Supervisor.
• Use and/or prepare equipment adhering to aseptic technique.
• Participate in personal and professional growth and development.
• Perform other duties as assigned by the registered nurse.
• Provide health care instructions to the patient per assessment and plan.
