Tennessee Quality Care in Memphis, TN is hiring a Full Time Licensed Practical Nurse LPN to provide one-on-one home health care throughout Shelby County and the Memphis area. As a Home Health LPN, you’ll deliver skilled nursing care in patients’ homes, work closely with the RN and care team, complete visits, monitor

Qualifications

• Current Tennessee LPN/LVN license is required.
• CPR certification is required.
• Reliable transportation and a valid driver’s license with auto insurance are required.
• 3 years nursing experience preferred, Home Health or Medical-Surgical experience preferred.

Responsibilities

• Provide direct patient care in patients’ homes per the established plan of care.
• Complete skilled nursing visits.
• Administer treatments and medications as ordered.
• Monitor patient status and report changes to the RN.
• Educate patients and families on medications, treatments and self-care.
• Complete accurate clinical documentation.
• Follow infection control and safety procedures.
• Collaborate with RN, physicians, therapists and care team.
• Support patients in managing chronic conditions and maintaining independence.
