Tennessee Quality Care in Shelby County, TN is seeking a Licensed Practical Nurse (LPN) to join our Home Health team full-time. You will provide hands-on, compassionate care in patients' homes and coordinate with an interdisciplinary team to support healing and independence.

Qualifications

• Active LPN/LVN license in the state
• Graduate from an accredited practical/vocational nursing program
• 3+ years of nursing experience (Home Health preferred)
• Current CPR certification

Responsibilities

• Deliver direct patient care in the home per state Nurse Practice Act
• Implement and follow care plans created by the RN
• Educate patients and families on self-care and treatment plans
• Document care accurately and timely
• Coordinate with RN and care team to ensure comprehensive support
• Adhere to infection control and safety protocols

What You’ll Do

As a Home Health LPN, you’ll provide hands‑on, compassionate care while working closely with an interdisciplinary team:

• Deliver direct patient care in accordance with the state Nurse Practice Act
• Carry out care plans developed by the Registered Nurse
• Educate patients and families on self‑care and treatment plans
• Document care accurately and timely
• Coordinate with RN and care team to ensure patients receive comprehensive support
• Utilize proper techniques for equipment, supplies, and infection control
• Support patients’ physical, emotional, and educational needs in the home

What You’ll Need

• Active LPN/LVN license in the state
• Graduate of an accredited practical/vocational nursing program
• 3+ years of nursing experience (Home Health, Med‑Surg, or Community Health preferred)
• Current CPR certification
• Reliable transportation, valid driver’s license, and auto insurance
