Job summary

Adoration Home Health in Tellico Plains, TN is seeking a licensed practical nurse for a PRN, home health role. You will provide patient-centered 1:1 care, administer medications, and document care while coordinating with the team and physicians.

Ideal candidates hold an LPN/LPT/LVN license, have a valid driver's license, and CPR certification. Home health experience is preferred and driving to patient homes is required.

Qualifications

• LPN/LPT/LVN degree with current unrestricted license in state of practice.
• Valid driver’s license and car insurance.
• CPR certification; home health experience preferred.

Responsibilities

• Provide 1:1 care following the client’s plan of care.
• Administer medications, feedings, oxygen, and ostomy care as needed.
• Document nursing actions and communicate with family and physicians.
• Coordinate supplies, medications, and feedings to ensure availability.
• Ensure patient needs are met per physician instructions.

Responsibilities

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home Health care experience preferred
