Job summary

Lake Wales Wellness & Rehabilitation Center in Lake Wales, FL is seeking a compassionate LPN to join our care team. You will oversee daily CNA activities, participate in admissions and transfers, and administer medications under physician orders.

The role requires an unrestricted Florida LPN license and CPR/BLS certification, with preference given to those with long-term care experience. This is a full-time, on-site position supporting residents and staff.

Qualifications

• Current Florida LPN license is required.
• CPR/BLS certification required.
• Experience in long-term care is preferred.
• Ability to supervise CNAs and coordinate resident care.

Responsibilities

• Direct the daily activities of the CNAs and ensure policies and procedures are followed by nursing personnel.
• Participate in admissions, discharges and transfers of residents as required.
• Assess residents' conditions, prepare and administer medications per physician orders.
• Initiate, review, and update care plans.
• Report any changes in a resident’s condition to the nurse supervisor and keep them informed of factors that interfere with performance.
• Perform other assigned duties given by clinical or administrative leadership.
• Maintain confidentiality of protected health information, including verbal, written and electronic communications.

LPN Position Summary
