Lakeside Center for Rehabilitation & Healing in Jacksonville, FL is hiring a Licensed Practical Nurse (LPN) to join a caring, supportive team. Shifts available include 7:00 AM–7:00 PM and 7:00 PM–7:00 AM at 11411 Armsdale Road, Jacksonville, FL 32218.

The role includes admissions, medication administration, infection control, and collaborating with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• LPN license in good standing (state or multi-state).
• CPR certification required.

Responsibilities

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians; medication administration.
• Follow infection control, emergencies, and center policies.
• Collaborate with the resident's interdisciplinary team to achieve best outcomes.
• Report abnormal changes in residents' condition as per policy.
• Provide long-term or rehabilitative nursing care as directed.
• Collect resident data, observe and report pertinent information.
• Implement and evaluate the patient/resident plan of care per state regulations.
• Supervise care given by other nursing staff when applicable.
• Maintain knowledge of documentation requirements and equipment setup.

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State or Multistate License
• CPR Certification required
• Two Years prior nursing experience preferred
