Wellbound Certified Home Health Agency is seeking a dedicated Field Nurse to provide compassionate nursing care to patients in assisted living facilities and their homes across New York. You will assess health conditions, implement care plans, administer medications, and support families in managing chronic conditions.

This role requires NY RN licensure, strong communication, independence, and willingness to travel between patient homes. Pediatric experience is a plus.

• Must hold a valid NY RN license.
• Experience in home health care or similar setting preferred.
• Strong communication skills with patients and families.
• Ability to travel between patient homes and manage time efficiently.

Responsibilities

• Deliver high-quality nursing care to patients in their assisted living facility, focusing on both clinical and functional needs.

• Conduct thorough assessments of patient health conditions and develop individualized care plans.

• Administer medications and treatments as prescribed by physicians.

• Educate patients and families on health management and self-care techniques.

• Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
