This role requires travel between patient homes and strong independent work.

Qualifications

• Valid NY nursing license and current licensure.
• Experience in home health care or related settings preferred.
• Strong communication with patients and families.
• Ability to work independently and travel to patient homes.
• Experience working with children is highly desirable.

Responsibilities

• Deliver high-quality nursing care to patients in assisted living facilities.
• Conduct comprehensive health assessments and create care plans.
• Administer medications and treatments as prescribed.
• Educate patients and families on health management and self-care.
• Coordinate with interdisciplinary teams for holistic care.
• Monitor patient progress and adjust plans accordingly.
• Insulin injections and wound care as required.
