Job summary

Envision Health Inc. is seeking a Licensed Practical Nurse (LPN) to join our healthcare team in Virginia. You will provide quality patient care, take vitals, collect specimens, and assist with care plans across the MH-NP and RN teams.

Required Virginia LPN license and BLS certification; psychiatric/mental health experience is a plus. You will help coordinate care, monitor patient status, and ensure accurate documentation in a multidisciplinary setting.

Qualifications

Responsibilities

Duties

• Work along side our MH-NP with scheduling appointments, maintaining records
• Perform vitals on each client and share with NP on a timely manner

• Work alongside R.N. to develop and maintain Lab orders and specimen.
• Collaborate with healthcare professionals to develop and implement patient care plans
• Monitor patients' conditions and report any changes to the healthcare team
• Follow-up care coordination with individuals following D/C from hospital / Rehab

Qualifications

• Valid LPN license in Virginia
• Experience in psychiatric/mental health is a plus
• Proficient in physical examination techniques and specimen collection processing
• Basic life support (BLS) certification required
