Job summary

Community Health Alliance-Ohio is seeking experienced LPNs to provide medically monitored nursing care in a Level 3.7 Withdrawal Management residential facility in Hamilton, Ohio. You will perform assessments, monitor withdrawal symptoms, administer medications, document care in the EHR, and collaborate with an interdisciplinary team to support recovery with compassion and trauma-informed care.

Qualifications

• Licensed in Ohio as LPN.
• Two years related experience in withdrawal management, behavioral health, residential treatment, or acute care preferred.
• Current BLS/CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Complete nursing tasks as directed by a RN or medical provider.
• Provide medically monitored withdrawal management per ASAM 3.7 standards.
• Conduct nursing assessments including vitals and withdrawal scales.
• Administer medications safely and accurately per protocol.
• Coordinate care as part of an interdisciplinary team.
• Monitor patient appointments and collect health data for medical provider.
• Educate patients on monitoring symptoms and health conditions.
• Collect specimens for laboratory testing as needed.
• Document care in the electronic health record (EHR) in compliance with regulations.
• Practice within the Ohio Board of Nursing scope of practice.

LPN's starting pay is $30.00 per hour with a $2.00 per hour shift differential.What You'll Do:As an LPN, you will:Complete nursing tasks as directed by a registered nurse or medical provider.Provide medically monitored withdrawal management in accordance with ASAM 3.7 standards.Conduct nursing assessments, including vitals, withdrawal scales (e.g., CIWA/COWS), and ongoing symptom monitoring.Administer medications safely and accurately, including PRN withdrawal medications per protocol.Participates as part of an interdisciplinary team coordinating patient care.Monitors patient appointments and collects health information and vitals for medical provider.Educates patients on monitoring and managing symptoms, medication and health conditions.Collects blood, urine and other specimens as needed for laboratory testing.Administers injections, TB testing, vaccinations as ordered by medical provider.Completes health and symptomology screenings to aid in patient care.Coordinates with other medical facilities to obtain medical records for treatment of patients.Implements appropriate nursing interventions and actions according to established plan of care within scope of practice.Support clients through the withdrawal process with empathy, professionalism, and therapeutic communication.Document all care in the electronic health record (EHR) in compliance with state and federal regulations.Practice within the scope of practice as outlined by the Ohio Board of Nursing.Licensed in the State of Ohio as a Licensed Practical Nurse (LPN).Two years related experience in withdrawal management, behavioral health, residential treatment, or acute care preferred.Current BLS/CPR certification required.Valid driver's license and reliable transportation (travel between sites required).Ability to work flexible hours, including early morning, weekends or evenings if needed.Experience in substance use treatment, behavioral health or related nursing field preferred.Knowledge of medication-assisted treatment (MAT), harm reduction and trauma-informed care is preferred.Commitment to providing nonjudgmental, empathetic care to individuals with substance use and behavioral health disorders.Effective verbal and written communication skills.Proficient with Microsoft 365 applications, including Office Suite, Outlook and Teams.Adept at learning and navigating electronic health record systems.Community Health Alliance is a leading provider of behavioral health and human services, delivering high-quality mental health and substance use treatment across a comprehensive continuum of care.
