Job summary

Silvercrest Rehabilitation in Crestview, FL seeks a dedicated LPN to join our faith-based, resident-focused team. You will deliver medications and treatments, monitor conditions, and support daily living activities to ensure comfort and safety for residents.

We value clear documentation, collaboration with interdisciplinary teams and families, and strict adherence to infection control. Prior SNF/LTC experience is a plus, and a valid LPN license is required.

Qualifications

• Active LPN license in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting is preferred but not required.

Responsibilities

• Deliver medications and treatments along with supervision of Medication Aids where applicable
• Monitor resident conditions and respond quickly to changes, ensuring safety, comfort, and continuity of care.
• Support activities of daily living
• Maintain clear, timely documentation
