Position Title: Licensed Practical Nurse – LPN

Department: Behavioral Health

Shift: Days

Status: Full-time

Schedule: Twelve-Hour Shifts

Compensation: $27.00 - $33.00

The estimated range is the budgeted amount for this position. Final offers are based on various factors, including skill set, experience, location, qualifications and comparisons to colleagues in similar roles.

Position Summary

The Licensed Practical Nurse provides quality patient care utilizing the nursing process under the direct supervision of a Registered Nurse and/or Licensed Physician.

Essential Functions
  • Participates in assessment/reassessments of patient(s) by observing, collecting, recording and reporting objective and subjective data in an accurate and timely manner. Reports the condition and/ or change in condition of the patient(s) as well as signs and symptoms that deviate from normal health status.
  • Assists in developing the nursing plan of care.
  • Assists in implementing the nursing plan of care within the concepts of the Licensed Practical Nurse curriculum with consideration for safety in practice; according to established prioritization of needs; documenting and communicating nursing interventions and responses of patient(s) to care.
  • Assists in evaluating patient(s) responses; documents and communicates evaluation data to appropriate health care team members; contributes to the modification of the nursing plan of care on the basis of ongoing evaluation of the patient(s).
  • Collaborate with team members by sharing relevant information, demonstrating initiative and supporting team decisions and the work life of the department.
  • Performs other related duties as assigned within the scope of practice after demonstrating required competencies or completing requisite training.
