Job summary

Hall of Fame Healthcare Center in Canton, OH seeks an LPN with empathy and patience to care for residents. This full-time position covers 7am–7pm on day shifts, with responsibilities including assessments, admissions, discharges, wound care, and supervising CNAs. LPN license required and strong teamwork is essential.

Hall of Fame Healthcare Center is seeking an LPN with genuine empathy and patience to help care for our residents’ needs. We are hiring for full-time 1st shift (7am-7pm).

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
