Traditional Home Health Care in Auburn, PA is seeking a compassionate Licensed Practical Nurse (LPN) to join the Pediatric/Adult Skilled Division for in-home care. This role covers pediatric and adult patients in their own homes, with opportunities for one-on-one patient interaction and meaningful support.

The position offers flexible scheduling with an immediate shift opening (2 nights/week, 8:30pm-5:30am) and a temporary day shift option (Mon-Fri 5:30am-4:30pm).

Responsibilities

• One to One patient care in home/school environment
• Implement and document the plan of care to ensure quality and continuity of care for patient
• Administration of prescribed medication, treatments and therapies
• Patient assessments; coordination of care
• Reports any changes in the client condition to Clinical Manager and physician
• Health promotion, teaching, and training of family members
