Job summary

Qualifications

• Master's degree in counseling or social work is required.
• Current licensure as LPC or LCSW in Mississippi is required.
• Knowledge of psychosocial assessment and counseling methods.

Responsibilities

• Conducts and documents comprehensive psychosocial assessments to evaluate patients' needs.
• Performs Level II transplant evaluations to determine discharge planning needs.
• Maintains documentation, prepares reports, and ensures compliance with guidelines.
• Reassesses patient needs and updates the plan of care based on progress.
• Provides delivery of care plan interventions, including crisis intervention and advocacy.
• Collaborates with the healthcare team to develop integrated care plans.
• Identifies community resources, makes referrals, and facilitates access to services.
• Participates in performance improvement initiatives and attends meetings.
