Qualifications

• Master's degree in counseling or social work.

• Experience with psychosocial assessments and counseling.

Responsibilities

• Conducts comprehensive psychosocial assessments to evaluate patients' social, emotional, environmental, and medical needs.
• Performs Level II evaluations for transplant applicants, donors, and individuals with mental illness or intellectual disabilities.
• Maintains thorough documentation, prepares reports, and ensures compliance with guidelines and standards.
• Continuously reassesses patient needs and updates the plan of care based on progress.
• Ensures access to medical coverage by securing necessary documentation and supporting materials.
• Provides or supervises delivery of care plan interventions including crisis intervention, bereavement support, counseling, education, and advocacy.
• Collaborates with the interdisciplinary team to develop and implement care plans.
