Job summary

Total Life, Inc. is seeking a Longevity Ambassador to represent us in Greater Philadelphia through in-person wellness assessments for older adults. You will act as the local contact at partner sites, building relationships with physician practices and residential communities across your territory.

You will host longevity screening events, engage attendees, and help interested seniors book their first appointment, while coordinating with the virtual therapy team for smooth care transitions.

Qualifications

• Extroverted and cheerful personality
• Comfortable initiating conversations with new people
• Experience or interest in community outreach with older adults

Responsibilities

• Conduct in-person wellness assessments for older adults to evaluate emotional, cognitive, and social well-being.
• Represent Total Life in the community and residential partner sites as the local point of contact for clients, caregivers, and partners.
• Build and maintain referral relationships with physician practices, senior living communities, and home health agencies across your territory.
• Host Total Life’s longevity screening events at partner sites, including setup and attendee engagement.
• Collaborate with our virtual therapy team to ensure smooth transitions into ongoing care.
• Maintain accurate and timely documentation, and log all community outreach activity in our CRM the same day.
• Participate in team meetings and contribute to improving our community-based programs.
