Job summary

Cayuga Nursing & Rehab is seeking a full-time Social Worker to provide comprehensive social services for residents and support their well-being.

The role involves liaising with medical staff, families, and outside agencies; interviewing residents for social histories; and assisting with admissions, discharges, and care-plan discussions.

Qualified candidates will hold a Bachelor's degree in social work and have experience in long-term care; familiarity with Point Click Care is a plus.

Qualifications

• Bachelor's degree in social work or closely related field required.
• Experience in long term care preferred (1-2 years minimum).
• Familiarity with Point Click Care preferred.

Responsibilities

• Providing social services for residents
• Acting as a liaison between medical staff, families, and outside agencies.
• Interviews residents and families for social histories
• Ensures that social and personal aspects of resident care are properly managed
• Collects and updates information on community health and welfare resources
• Assist with admissions and discharges
• Conduct care plan meetings & discharge meetings
