Job summary

Cayuga Nursing & Rehab is seeking a full-time Social Worker to provide comprehensive social services for residents and support their well-being.

The role involves liaising with medical staff, families, and outside agencies; interviewing residents for social histories; and assisting with admissions, discharges, and care-plan discussions.

Qualified candidates will hold a Bachelor's degree in social work and have experience in long-term care; familiarity with Point Click Care is a plus.

• Experience in long term care preferred (1-2 years minimum).
• Familiarity with Point Click Care preferred.
