Job summary

Geneva Lake Manor in Lake Geneva, Wisconsin seeks a full-time Social Worker to support residents’ emotional and psychosocial needs under the Nursing Home Administrator. You will coordinate care plans, liaise between departments, and work with discharge planning and resources to meet resident goals in compliance with state and federal regulations.

Qualifications

• Bachelor's degree in Social Work or Human Services (sociology, Rehab Counseling, Psychology)
• Minimum one-year experience as a Social Worker in a long-term care setting (preferred)
• Knowledge of and ability to obtain community resources
• Demonstrated excellent oral and written communication skills.
• Fluent in English with ability to read, write and speak English
• Commitment to a multi-disciplinary approach to resident care
• Adherence to state and federal regulations
• Flexibility with schedule to meet resident needs

Responsibilities

• Demonstrates professional social work practice and applies standards to job situations.
• Keeps abreast of state and federal regulations and advises administration on changes.
• Develops and maintains customer-friendly service with all departments.
• Ensures social service programs meet resident needs.
• Maintains confidentiality of resident information and rights.
• Provides annual in-service on resident rights and confidentiality.
• Collects data from interviews for MDS, billing and care planning.
• Coordinates resident care plan meetings with the multidisciplinary team.
• Provides staff and community consultations to address resident needs.
• Offers educational opportunities for residents and families.
• Builds relationships with community health and social agencies.
