Job summary

Catholic Charities of Tri-County Services is seeking a Long Term Care Ombudsman Program Advocate in Schenectady, NY. The role focuses on resolving resident concerns, upholding resident rights, and addressing systemic issues within long-term care facilities.

The position requires an Associate's Degree in Human Services or related field and 3–5 years of diverse population experience, plus prior long-term care exposure. A 36-hour NY certification is mandatory and ongoing education is provided.

Qualifications

  • Previous experience with Long-term care facility visits.
  • Completes New York State mandated 36-hour certification training and attends ongoing in-service trainings.

Responsibilities

  • Responds to resident issues and concerns by phone, email, and in-person visits at facilities.
  • Advocates for resident rights, addressing dissatisfaction, avoiding elder abuse, and ensuring person-centered care.
  • Conducts in-person routine and complaint visits to facilities and initiates ombudsman actions when needed.
  • Meets with facility administrators and staff to resolve concerns within assigned service area.
  • Ensures confidentiality of Ombudsman Program files and protect resident identity as required by law.
  • Participates in ongoing education and public awareness to address systemic issues and elder abuse.
  • Presents the program's services in public forums and community events as assigned.

As the Long Term Care Ombudsman Program Advocate, you will play a pivotal role in individuals' lives, making a significant impact through:

  • Responds to resident issues and concerns by phone, email and in-person visit to residents at the facilities in which they reside.
  • Advocates for the upholding of resident rights including addressing resident dissatisfaction or complaints, using a person centered and directed manner, and the avoidance of elder abuse, mistreatment and neglect.
  • Conducts in-person routine and complaint related visits to long-term care facilities and initiates ombudsman or multi resident complaint to address systemic issues as needed.
  • Completes New York State mandated 36-hour certification training and post certification attends periodic in service meetings to acquire minimum hours of annual continuing education.
  • Meets with facility administrators and other staff to resolve resident concerns and complaints, as necessary within assigned service area.
  • Assures that any files maintained by the Ombudsman Program are disclosed only at the discretion of the State Long Term Care Ombudsman, and that the identity of any complainant or resident of a long-term care facility is not disclosed unless as specified by law.
  • Participates in an ongoing program of education and public awareness to address systematic problems and elder abuse as well as the program’s need to recruit Ombudsman volunteers.
  • Presents the program’s services in public forums, such as speaking engagements, agency fairs, as assigned by Program Director.

Long Term Care Ombudsman Program Advocate Salary: $ 25.00

Long Term Care Ombudsman Program Advocate Schedule: Mon-Fri 8:30AM - 4:00PM (35 hours/week)

Qualifications:

  • Associate's Degree in Human Services or a related field
  • 3 - 5 years of experience working with a diverse populationi>
