Qualifications

• Bachelor’s degree in a human service-related field is required.
• At least one year of full-time health or human service experience preferred.
• Strong listening, interviewing and advocacy abilities are essential.
• Knowledge of long-term care options and public benefits is preferred.
• Ability to organize work and communicate clearly, orally and in writing.
• Proficiency with computers and documentation systems is expected.
• Certification as a Long-Term Care Functional Screener is preferred.

Responsibilities

• Provide information and assistance on long-term care resources.
• Help clients access Medicaid, Medicare and other programs.
• Interview with individuals and families about care options.
• Document calls and maintain case records and enrollment docs.
• Attend trainings, meetings and maintain confidentiality.
