Job summary

Sweetwater Care seeks a licensed LVN in California to provide nursing care within scope of practice, following physician orders and center policies. The role includes medication administration, treatments, and coordinating patient care with physicians and families.

Candidates with long-term care experience and Point Click Care charting familiarity are preferred. One year of nursing management experience is nice to have, but not required; strong leadership and teamwork are essential.

Qualifications

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual
• Understands, participates and assists in Quality Improvement Program
• Utilizes Quality Life Manual for patient placement and monitoring
• Notifies departments of patient changes (room changes, LOA, new admits, discharge)
• Conducts family and patient education
• Administers medications timely and per facility policy
• Administers treatments timely and per facility policy
• Receives, transcribes, and carries out physician orders when allowed by the Nurse Practice Act
• Makes rounds with physicians when necessary

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
