Job summary

Sweetwater Care is seeking an LVN in California to provide nursing care in accordance with standards and physician orders. You will support quality improvement and demonstrate strong clinical judgment in a long-term care setting.

You will administer medications and treatments timely, transcribe physician orders as allowed, and collaborate with physicians for rounds and patient education, maintaining compliance with center policies and procedures.

Qualifications

• Licensed LVN in California
• Long-term care experience
• Nurse management experience preferred (not required)
• PointClickCare charting experience

Responsibilities

• Provides nursing care within scope of practice and follows standards, physician orders, and center policies.
• Maintains infection control practices per the Company Infection Control Manual.
• Participates in and supports the Quality Improvement Program.
• Utilizes the Quality Life Manual for patient identification, placement, and monitoring.
• Notifies other departments of patient changes (room changes, LOA, admits, discharge).
• Educates patients and families.
• Administers medications in a timely manner per facility policy.
• Administers treatments in a timely manner per facility policy.
• Receives, transcribes, and implements physician orders per Nurse Practice Act.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
