Job summary

Sweetwater Care in California is seeking an LVN who will provide nursing care within the scope of practice, following physician orders and facility policies to meet patients' needs. The role includes coordinating rounds with physicians, administering medications and treatments timely, and nursing management activities.

Long-term care experience and PointClickCare charting familiarity are preferred. The position requires current California LVN licensure and a willingness to participate in quality

Qualifications

• Active California LVN license.

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for patient identification and placement.
• Notifies other departments of patient changes (room changes, LOA, admits, discharge).
• Conducts family and patient education.
• Administers medications timely according to facility policy.
• Administers treatments timely according to facility policy.
• Receives, transcribes, and carries out physician orders as allowed.
• Makes rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
