Job summary

Sweetwater Care is seeking a skilled LVN supervisor to provide nursing care within the scope of practice and ensure patient needs are met according to standards, physician orders, and state guidelines.

The role requires experience in long-term care and familiarity with Point Click Care charting to document orders, rounds with physicians, and coordination with hospital and family members. Strong leadership and adherence to policies are essential.

Qualifications

• Currently licensed as LVN in California.
• One-year prior nursing management experience preferred, not required.
• Long term care experience.
• Point Click Care charting experience.

Responsibilities

• Maintains knowledge and use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Uses Quality Life Manual for patient placement and monitoring.
• Notifies other departments of patient changes (room changes, LOA, admits, discharge).
• Educates families and patients.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
