ConnectedHealth is seeking a Certified Nursing Assistant to provide hands-on nursing assistance in a long-term care facility near Brush, Colorado, during evening shifts. The CNA will support daily living activities, monitor health changes, document care, and contribute to resident well-being and team efficiency.

Responsibilities include ADLs assistance, vital signs, mobility support, accurate documentation, and collaboration with nursing staff to ensure high-quality, resident-centered care.

Qualifications

• Active CNA certification eligible to practice in Colorado.
• Completion of a state-approved CNA training program and competency requirements.
• Ability to work evening shifts and every other weekend.
• Must live within one hour of the facility.
• Professional references and background checks required.
