Job summary

Sweetwater Care in Anaheim, CA is seeking an experienced LVN to lead nursing operations and ensure high standards of patient care under physician orders and local regulations.

The role requires California LVN licensure, long-term care experience, and at least one year in nursing management; PCC charting experience is a plus. You will supervise staff, participate in infection control, and drive quality improvement initiatives to enhance resident outcomes.

Qualifications

• Licensed LVN in California; compact license if working in a compact state.
• 1 year of nursing management experience preferred.
• Long-term care experience required.
• PointClickCare charting experience preferred.

Responsibilities

• Demonstrates knowledge and appropriate use of Infection Control Manual.
• Participates in Quality Improvement Program.
• Uses the Quality Life Manual for patient identification and monitoring.
• Notifies departments of patient changes, admissions, discharges, etc.
• Conducts family and patient education.
• Administers medications and treatments timely per facility policy.
• Receives, transcribes and carries out physician orders as allowed by Nurse Practice Act.
• Makes rounds with physicians when necessary.

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
