Responsibilities

• Assists to develop coordinated care plans for patients with complex medical and/or behavioral health needs.
• Identify outreach, wellness and education planning needs of identified members.
• Coordinate referrals between physical and behavioral providers to community resources.
• Provide outreach to community-based and county transportation to assist members.
• Work with payers to apply member benefits and assist the care team.
• Maintain documentation for patient care activities.
• Evaluate processes to improve care coordination programs.
• Use communication and collaboration skills in social work practice.
• Perform social work activities across PCP offices, homes, hospital, and community settings.
• Develop relationships with community agencies for program eligibility and referrals.
• Utilize conflict resolution and advocacy in patient movement through care continuum.
• Partner with care coordination programs to facilitate referrals.
• Participate in required training to maintain licensure.
• Use evidence-based screening to identify barriers and needs affecting patient function.
