FMOL Health’s Our Lady of the Lake is seeking a Social Worker LMSW to conduct psychosocial evaluations, manage care plans, and participate in discharge planning. You will guide families through long‑term care placement, coordinate with facilities, and advocate for timely access to services.

The role emphasizes collaboration with physicians, clinics, and community partners, ensuring seamless transitions and comprehensive client support.

Qualifications

• Master's degree required.
• Crisis Intervention Training within 90 days of employment.
• Proficient in English (verbal and written).
• Current Louisiana State Board license (CSW or LMSW).

Responsibilities

• Meet individuals and families to assess long‑term care needs and discuss nursing home placement options.
• Conduct psychosocial assessments and educate clients on placement process and resources.
• Coordinate referrals and admissions to nursing homes and long‑term care facilities.
• Advocate for timely access to services and maintain regular communication with stakeholders.
• Develop and maintain referral relationships with physicians, clinics, rehabilitation centers, home health agencies, hospice providers, and other organizations.
• Identify barriers to placement and develop actionable solutions with clients and partners.
• Gather required documentation and complete placement-related paperwork.
• Maintain confidential case records in line with regulatory requirements.
