Job summary

LCMC Health is seeking a social worker to serve as a pivotal member of the multidisciplinary care team in New Orleans, LA. You will assess psychosocial needs, counsel patients and families, and support discharge planning for complex medical cases.

You will collaborate with physicians, nurses, and social work staff to identify barriers, provide resources, and advocate for patient rights and advance directives throughout the care journey.

Qualifications

• Licensed Master Social Worker (LMSW) or eligible; LMSW must be obtained within 6 months of hire.
• MSW degree required or equivalent.
• Ability to conduct psychosocial assessments and counseling.
• Experience working with multidisciplinary teams and discharge planning.

Responsibilities

• Assess psychosocial needs via patient and family interviews and chart review.
• Evaluate coping, support systems, and barriers affecting illness and discharge plans.
• Identify barriers and plan interventions to support treatment goals.
• Document findings, plan, and interventions in medical records.
• Provide crisis intervention and supportive counseling as needed.
• Assess cases of elder/child/sexual or domestic abuse or neglect.
• Refer to community services such as palliative or hospice when appropriate.
• Facilitate family meetings and coordinate with agencies for decision making.
• Identify complex social/medical issues and escalate with the manager.
• Review caseload and share progress and barriers with the care team.
• Assess financial assistance needs and guide to resources.
• Understand payor requirements for post-hospital services and resources.
• Educate patients/families about patient rights and advance directives.
