The role conducts comprehensive assessments, provides emotional support, and collaborates with the interdisciplinary care team to develop and implement effective discharge plans.

The LMSW will play a key role in improving patient outcomes, reducing readmissions, and supporting vulnerable populations

Qualifications

• Master's Degree in Social Work (MSW) or Psychology required.
• 2-4 years of healthcare or clinical social work experience preferred.

Responsibilities

• Conducts age-appropriate psychosocial assessments for patients across the lifespan.
• Assesses psychosocial factors to identify barriers to recovery and discharge.
• Provides emotional support, crisis intervention, and counseling to patients and families.
• Identifies social, emotional, and financial needs and facilitates referrals.
• Develops and follows through on individualized treatment and discharge plans with the team.
• Supports transition of care by coordinating services such as home health, hospice, and long-term care placement.
• Maintains timely documentation per regulatory standards.
