Job summary

Housing Works Community Healthcare (HWCH) seeks a licensed Master Social Worker (LMSW) to provide integrated behavioral health services within the HUB Program and the East New York Primary Care Clinic. This hybrid role allocates 50% of time to HUB activities focused on HIV prevention, harm reduction, and behavioral health integration, and 50% of time to outpatient mental health counseling within ENY.

Qualifications

• Active New York State LMSW required.
• Minimum 2 years of clinical behavioral health experience.
• Experience with marginalized populations (LGBTQ+, people living with HIV).
• Experience with trauma-informed counseling and crisis intervention.
• Experience with electronic health records (eCW preferred).

Responsibilities

• Provide integrated behavioral health services in HUB and ENY clinics.
• Conduct mental health and substance use screenings using validated tools.
• Deliver brief interventions (MI, CBT-informed) and crisis support.
• Collaborate with navigators, outreach, and medical providers for HIV care.
• Maintain case documentation and participate in supervision and team meetings.
