Housing Works Community Healthcare (HWCH) seeks a compassionate, clinically skilled Licensed Master Social Worker (LMSW) to provide integrated behavioral health services within the Hook Up and Bridge (HUB) Program and the East New York Primary Care Clinic. This hybrid role allocates 50% of time to HUB program activities focused on HIV prevention, harm reduction, and behavioral health integration, and 50% of time to providing outpatient mental health counseling services within the ENY primary care clinic. The LMSW will support individuals disproportionately impacted by HIV, substance use, housing instability, trauma, and structural barriers to care.
The LMSW will provide evidence-based brief interventions, mental health screening, crisis support, and counseling using Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT)-informed interventions, trauma-informed care, and harm reduction principles. The position plays a key role in improving engagement in HIV prevention and treatment services, supporting behavioral health stabilization, and strengthening linkage to ongoing mental health care.
