Qualifications

• Master's Degree in Social Work (MSW) or Psychology required.
• 2-4 years of experience in a healthcare or clinical social work setting preferred.

Responsibilities

• Conducts age-appropriate psychosocial assessments for patients across the lifespan.
• Assesses psychosocial factors to identify barriers to recovery and discharge.
• Provides emotional support and counseling to patients and families.
• Facilitates referrals to internal and external support services.
• Develops individualized treatment and discharge plans with the interdisciplinary team.
• Supports transition of care including home health, hospice, and long-term care placement.
• Maintains timely documentation in accordance with standards.
• Collaborates with physicians, nurses, case managers, and others on care planning.
