Job summary

Mille Lacs Health System is seeking a Hospital Social Worker (LSW/LMSW) to provide case management, discharge planning, and psychosocial support across acute and chronic care settings. The role emphasizes addressing social determinants of health and coordinating timely transitions of care with the care team.

Responsibilities include community referrals, education, and collaboration with families, physicians, and administrators.

Qualifications

• Bachelor's degree in Social Service field required.
• Must be a licensed Social Worker in the State of Minnesota.
• Clinical skills, ability to work productively and effectively, ability to function well as a member of a multidisciplinary team, and ability to represent the program well in interaction both inside and outside the host hospital.
• Knowledge of age specific need including, but not limited to: life stage needs (grief, losses, end of life, etc.), severity of illness/intensity of service criteria, typical concurrent medical issues and continuity of care needs.
• Must be trained in de-escalation techniques, or able to obtain certifications within 90 days of hire.
• Must have knowledge of EMR.
• Certifications: BLS Required.
• Computer skills - Outlook, Excel, PowerPoint, etc.
• Preferred Education and Experience - Master's degree as a Social Worker preferred, with 5 years mental health experience, one field placement experience or at least one year experience working with Adult Psychiatric patients, and master's level social worker supervision.

Responsibilities

• Engages in timely, efficient discharge planning.
• Works in close conjunction with patient, family, nursing staff and attending physician.
• Shows knowledge of referral/placement process and available community resources.
• Collaborate and assist other departments as needed.
• Communicate effectively with families and agencies, conveying a positive professional image.
• Brings to Program Director's attention significant issues related to program functioning.
• Coordinates periodic social work communication/networking/quality sessions with other MLHS staff engaged in discharge planning and social work duties.

Education

Bachelor's degree in Social Service field

Master's degree in Social Work (MSW) preferred
