Responsabilidades

• Conduct age-appropriate psychosocial assessments for patients across the lifespan.
• Assess psychosocial factors and barriers to recovery and discharge.
• Provide emotional support, crisis intervention, and counseling.
• Identify social, emotional, and financial needs and facilitate referrals.
• Develop and follow through on individualized treatment and discharge plans with the team.
• Manage high-risk referrals involving abuse, end-of-life care, or crises.
• Coordinate services such as home health, hospice, rehab, and long-term care.
