Qualifications

• Minimum one year of clinical social work experience in a health care setting.
• Master of Social Work (MSW) required.
• Licensure from multiple states; DC and MD/VA licensure expected within 6 months of hire.

Responsibilities

• Collaborates with health care team to coordinate cost-effective, appropriate services.
• Performs psychosocial assessments to inform care planning.
• Develops and monitors care plans with the nurse case manager to optimize functioning and quality of life.
• Identifies resources and facilitates access to community programs and supports.
• Documents case management activities consistently per standards and policies.
• Analyzes outcomes to drive program improvements in quality and cost effectiveness.
• Guides patients through local, state, and federally funded programs when appropriate.
• Provides counseling and education to patients, families, caregivers, and health plan staff; addresses end-of-life issues.
