Job summary

Bluestem Communities Management is seeking a licensed Masters Social Worker (LMSW) for Bluestem PACE in McPherson, KS. This full-time role focuses on organizing social work services for PACE participants and families, including assessments, counseling, and care coordination with the interdisciplinary team.

Responsibilities include conduct of initial assessments, ongoing care planning, regular reassessments, and active participation in team meetings.

Qualifications

• Master’s degree in social work from an accredited school of social work.
• Active LMSW in the state of Kansas.
• Current driver’s license and proof of auto insurance.
• Two years of experience on an interdisciplinary team in healthcare or community settings.

Responsibilities

• Perform in-person initial psycho-social assessments for enrollment and history.
• Collaborate with the interdisciplinary team to develop a comprehensive care plan.
• Conduct reassessments every six months or as needs change.
• Attend daily Interdisciplinary Team meetings and coordinate care planning.
• Provide liaison support with participants, families, and community agencies.
• Document all clinical services in medical records per policy.
• Assist with housing referrals, Medicaid eligibility, and transition planning.
